Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Premiere Recovery Center (“PRC,” “we,” “us,” or “our”) is committed to protecting the privacy of your medical and personal information. We create and maintain records of the care and services we provide to you. These records are used to support your treatment, coordinate care, obtain payment, and meet legal and regulatory requirements.

This Notice explains how we may use and disclose your protected health information (“PHI”), your rights regarding that information, and our legal responsibilities under applicable laws, including HIPAA and federal confidentiality regulations for substance use disorder treatment (42 CFR Part 2).

How We May Use or Disclose Your Health Information

Treatment

We may use and share your health information to provide, coordinate, or manage your care. This may include sharing information with clinicians, therapists, pharmacies, laboratories, or other providers involved in your treatment.

Payment

We may use and disclose your health information to obtain payment for services provided to you. This may include sharing information with insurance providers or other entities responsible for covering your care.

Health Care Operations

We may use and disclose your information to operate our programs, improve care quality, train staff, conduct audits, support compliance activities, and manage administrative functions.

Appointment Reminders and Communication

We may contact you to remind you about appointments, provide updates about your care, or share information related to your treatment.

Individuals Involved in Your Care

We may share relevant information with family members or others involved in your care, when appropriate and permitted by law, or when you authorize us to do so.

Business Associates

We may share information with third-party service providers who perform services on our behalf. These providers are required to protect your information under written agreements.

Required by Law

We may disclose your health information when required to do so by federal, state, or local law.

Public Health and Safety

We may disclose information to public health authorities or appropriate individuals when necessary to prevent or reduce a serious threat to health or safety, or as required for reporting purposes.

Legal and Administrative Proceedings

We may disclose information in response to court orders, subpoenas, or other lawful processes, as permitted by law.

Law Enforcement

We may disclose information to law enforcement when required or permitted by law.

Research, Audits, and Evaluations

We may disclose information for approved research, audits, or evaluations, in accordance with applicable laws and safeguards.

Additional Protections for Substance Use Disorder Treatment Information

Because PRC provides substance use disorder treatment services, certain information is protected under federal confidentiality regulations (42 CFR Part 2).

These regulations generally prohibit disclosure of patient-identifying information related to substance use disorder treatment without your written consent, except in specific circumstances allowed by law, such as medical emergencies, certain legal requirements, or approved audits and evaluations.

Any disclosure made under these rules may be subject to strict limitations on re-disclosure.

Uses and Disclosures That Require Your Authorization

We will not use or disclose your health information for purposes not described in this Notice without your written authorization, unless permitted or required by law. You may revoke your authorization at any time in writing, subject to certain limitations.

Your Rights Regarding Your Health Information

Right to Access

You have the right to inspect and obtain a copy of your health information, with limited exceptions.

Right to Request Amendments

You may request corrections or updates to your health information if you believe it is incomplete or inaccurate.

Right to Request Restrictions

You may request limits on how your information is used or disclosed. We will consider your request but may not be able to agree in all cases.

Right to Confidential Communications

You may request that we contact you in a specific way or at a specific location.

Right to an Accounting of Disclosures

You may request a list of certain disclosures of your health information made by PRC.

Right to a Copy of This Notice

You have the right to receive a paper or electronic copy of this Notice at any time.

Our Responsibilities

PRC is required by law to maintain the privacy and security of your protected health information, provide you with this Notice of our legal duties and privacy practices, and notify you following a breach of unsecured protected health information when required by law.

We will follow the terms of this Notice currently in effect.

Changes to This Notice

We may update this Notice from time to time. Any updates will apply to all health information we maintain and will be posted on this page with a revised effective date.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with PRC or with the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.
